Cocoa, Sedentary Behavior, and the Heart: What Flavanols Actually Protect
New studies from the University of Birmingham and the COSMOS trial reignite the debate over cocoa and cardiovascular health — but the answer is more nuanced than "eat chocolate for your heart."
Sitting still for just two hours is enough to measurably reduce blood vessel function — the so-called sedentary-induced endothelial dysfunction. That's the starting point of a University of Birmingham study, published in the Journal of Physiology and replicated in detail in Food & Function, which tested whether cocoa flavanols consumed before a prolonged sitting period could preserve flow-mediated dilation (FMD) — the main marker used to measure artery health.
The result: participants who consumed flavanol-rich cocoa, both the more and the less physically fit, didn't show the expected drop in arm and leg vascular function after the sitting period. According to the researchers, this is the first time this specific protective effect has been demonstrated in healthy young men.
This finding echoes — with an important caveat — the results of COSMOS, a large-scale randomized clinical trial in older adults, published in the American Journal of Clinical Nutrition. There, daily supplementation with cocoa flavanol extract (500mg/day, including 80mg of epicatechin) didn't produce a statistically significant reduction in total cardiovascular events, but it was associated with a 27% reduction in cardiovascular disease mortality — a secondary outcome the study authors themselves treat with caution, calling for confirmation in further research.
The Caveat That Changes the Reading
Both studies used cocoa or standardized extracts, with a known, controlled dose of flavanols — not a bar of chocolate off a store shelf. Flavanol content in cocoa varies drastically depending on fermentation, roasting, and, especially, the alkalization (dutching) used to soften the flavor of industrial cocoa — a step that can destroy much of these compounds. Bean-to-bar chocolate, processed with minimal intervention, tends to preserve more of that phenolic profile than standard cocoa powder — but that wasn't measured directly in the cited studies, and it doesn't mean a bar produces the same clinical effect as the lab-tested dose.
Even so, the findings point to something consistent: cocoa flavanols appear to have a real, measurable role in vascular function, even as the size and durability of that effect in everyday life are still being mapped out.
